District Nurses

District Nurses care for patients in the community. They look after patients who are housebound and too ill to come to the surgery.

They undertake blood samples, dressings, and post operative care. They also look after the management of patients with chronic diseases, diabetes, heart disease and asthma.

In addition they provide palliative care and support the informal carers at this time. They act as liaison for McMillan Nurses, Hospice at Home, Social Services and help to co-ordinate care in the discharge planning process.
