Staff Training

The practice will be closed every Thursday lunchtime between 12.30 and 13.30. This is for staff training. Telephone calls for urgent advice or requests for emergency care will be directed through to the duty GP.

On Wednesday 19th October and Thursday 17th November 2011 the practice will be closed between 1pm and 5pm for training.

Vasectomy Service

We operate a service where any man can either refer himself or get his GP to refer him for a No-Scalpel Vasectomy. This is a simple procedure undertaken in a local hospital clinic with an excellent success rate.

Pre-Registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
